Login Instructions
Enter Your Assigned Member ID, as found on your Activities Card.
- If you are a first time user, the default password is your last name in lower case.
- If you do not have a Membership ID, contact the Membership office in Anthem Center to be assigned one.
- Contact IT Support at 702-614-5873 for any log in issues.
- Click the Forgot Password link and enter your Member Number.
- You will receive an email with a link to reset your password. If you do not receive an email, please contact IT Support.
- All new passwords must be created using a complex password consisting of 8 characters or more in length, with at least one small letter, one capital letter, one number and one symbol.
